Why does TBC Healthcare need to collect and store personal data?

In order for us to provide you with a weight management service, we need to collect and store personal data for correspondence purposes and detailed service provision. This includes:

  • Your health history, including physical and mental health conditions, medications, and lifestyle information
  • Information about your disability or additional needs, including any reasonable adjustments required to support your participation in the programme
  • Your weight and clinical measurements recorded during the programme
  • Records of appointments, assessments, and clinical decisions

We are committed to ensuring that the information we collect and use is appropriate for this purpose and does not constitute an invasion of your privacy.

How we identify your records

When we send you online questionnaires or forms as part of your care, we use a unique system reference number (your PPS number) to identify your responses and link them securely to your patient record. This number is assigned to you when you are registered with our service and will be included in any correspondence asking you to complete a form.

You will be asked to enter this number when completing online questionnaires. This approach ensures your responses are matched to your records accurately and securely, without requiring you to enter sensitive personal information such as your full name or date of birth in an unsecured online form.

How we contact you

TBC Healthcare may contact you by telephone, email, SMS text message, or letter in connection with your care and appointments. We will not include sensitive health information in SMS messages. Where we use SMS to direct you to an online form or questionnaire, messages will not include personal health details. Your preferred method of contact can be discussed with your clinical team and will be recorded in your care record.

Reasonable adjustments

If you have a disability, long-term health condition, or additional need that affects your ability to engage with our programme, we will discuss and agree reasonable adjustments with you. Information about your disability and any adjustments made will be recorded in your clinical notes and taken into account throughout your care, including at any clinical review. This information is collected to fulfil our obligations under the Equality Act 2010 and to ensure you can access our services equitably.

Will TBC Healthcare share my personal data with anyone else?

We may pass your personal data to third-party service providers contracted to TBC Healthcare in the course of dealing with you, including:

  • Our clinical software provider (Rushcliffe / Private Practice Software) used to manage patient records
  • Online form and questionnaire platforms used to collect health information as part of your assessment and care
  • Other healthcare providers involved in your care, including your referring GP and, where applicable, NHS Trust teams

Any third parties with whom we share your data are obliged to keep your details securely and to use them only to fulfil the service they provide on our behalf. When they no longer need your data to fulfil this service, they will dispose of it in line with TBC Healthcare's procedures.

If we wish to pass your sensitive personal data to a third party in any other circumstance, we will only do so once we have obtained your consent, unless we are legally required to do otherwise.

We do not sell your personal information.

How will TBC Healthcare use the personal data it collects about me?
